Payment
Payment for services is generally due at the time services are rendered, unless other arrangements have been made. Payment may be made by cash or  check.

Insurance Plans I participate in most HMO or PPO provider panels.
I can prepare and submit claims for you to any insurance plan that permits you to chose “any willing provider.”  I accept payment through the Victim Witness Program (for California and other states) for established claims. I also can accept Workers Compensation.. I am not a contracting provider for Medicare, Medi-Cal or Medicaid.

In order to be of maximum service to the community my practice accepts most forms of insurance, including EAP (employee assisstance plans) and HMO and PPO plans by major US carriers.
